Study Links Saliva-Based Epigenetic Signals to OCD Treatment Outcomes

Obsessive-compulsive disorder (OCD) causes unwanted, recurring thoughts and repetitive behaviors that can severely disrupt daily life. More...

Exposure-based psychotherapy is among the most effective treatments, but the biological processes underlying treatment response remain poorly understood. Understanding these processes could help clarify why outcomes differ between individuals. New findings offer insight into this question, suggesting that epigenetic markers may be associated with how people with OCD respond to intensive psychotherapy.

Researchers from the University of Bergen (Bergen, Norway) studied DNA methylation in people with OCD receiving exposure-based psychotherapy. DNA methylation is an epigenetic mechanism in which small chemical groups attach to DNA and can influence gene activity without altering the genetic code. The work focused on whether methylation patterns were associated with response to the Bergen 4-Day Treatment, an intensive form of exposure-based psychotherapy.

The researchers analyzed saliva samples from approximately 900 people with OCD treated at eight clinics across Norway. Samples were collected before treatment, immediately after treatment, and at a three-month follow-up. This design allowed the investigators to examine both methylation patterns present before therapy and methylation changes occurring over time.

The study identified several DNA regions where methylation was associated with treatment response. Some differences were detectable before therapy or remained stable over time, with many showing strong genetic influences. Published in Neuropsychopharmacology, these findings suggest that certain epigenetic patterns may reflect relatively stable biological characteristics associated with how individuals respond to psychotherapy.

Changes in DNA methylation over time were also associated with treatment outcomes. Unlike the more stable methylation patterns, these changes showed no detectable genetic influences. They were also associated with other clinical factors, including medication use.

The identified methylation differences were located in or near genes involved in brain plasticity, stress responses, immune function, gene regulation, and cellular energy production. The findings highlight the potential of epigenetic markers to help predict individual responses to psychotherapy and support more personalized approaches to OCD treatment. However, the associations do not establish causal relationships, and further research is needed to determine whether these markers can reliably predict treatment outcomes in clinical practice.
